Challenges In most resource-limited countries, the follow-up of patients on antiretroviral therapy is mainly based on clinical examination and CD4 count. However, these tools are insufficient to identify and prevent situations of therapeutic failure. This limit leads to unnecessary or too late switching to rarer and more expensive second line treatments. The use of HIV viral load (CV) measurement is necessary to ensure appropriate treatment. The challenges In the absence of antiretroviral therapy (ART), the rate of mother-to-child transmission (MTCT) of HIV is 20-25% for HIV1 and 1-4% for HIV2. Breastfeeding carries an increased risk of MTCT during the first two months and in case of mixed feeding. With triple ARV therapy, MTCT rates of <2% are achieved. In 2009 in Niger, access to Prevention of Mother to Child Transmission (PMTCT) services is only 23% and access to ARV prescribing centres is often difficult. The issues The HIV/AIDS epidemic affects 1.6% of the adult population in Sierra Leone. The number of people living with HIV (PLHIV) was estimated at 52,000 in 2014. The epidemic is tending to stabilize, but the coverage of treatment needs is still too low. Significant disparities persist, with the epidemic having more of an impact on women, in urban areas and on the groups considered as being most at risk of infection, such as sex workers or men who have sex with men. Sierra Leone has implemented a national strategic plan to control HIV/AIDS, based on prevention, treatment and care.
